Job Description
The LPN, under the direction of a registered nurse, licensed physician, or other specified health care professional, participates in patient care commensurate to Scope of Practice\. This position will be working full time at Garfield County's Nursing Home, which is located at Garfield Memorial Hospital in Panguitch, Utah\.

Essential Job Duties
Patient Care:
+ May assist in the admission / check\-in process by orienting patient / significant other to patient care setting \(e\.g\., routines, patient care, and patient rights\)\. Assists with the completion of needed forms\.
+ Assists in the collection, reporting, and documentation of appropriate assessment data\. This may include taking and recording vital signs, height and weight, current medication information, and / or billing information\.
+ Identifies patient pain or discomfort and reports and responds appropriately\.
+ Assists patients with Activities of Daily Living\.
+ Prepares patient for medical and / or surgical medical interventions\.
+ Completes interventions within Scope of Practice\.
+ Assists physician or licensed independent practitioner with treatments / procedures\.
+ Obtains and tracks specimens for laboratory analysis \(e\.g\., venipunctures, stool, urine\)\.
+ Prepares, administers, and manages medication \(oral, IM, SQ, rectal, intradermal\)\.
+ Assists with stocking and coordinating supplies as needed\.
+ Appropriately documents: assessment data, interventions, physician / independent practitioner orders \(verbal and / or phone that fall within the LPN's Scope of Practice and must be signed by an RN\), update of care plan, update changes to discharge planning, medication administration, and patient and family education\.
+ Appropriately communicates with supervisory RN / physician / licensed independent practitioner regarding assessment findings, changes in patient condition, patient history data, implementation and / or changes to plan of care or discharge plan, and physician / independent practitioner orders \(verbal and / or phone\)\.
+ Participates in patient education\.
+ Integrates legal and ethical standards into practice including compliance with regulatory standards, practices within scope of licensure, provides accurate & timely documentation, and understands legal implications of care delivery\.
+ Demonstrates responsibility and accountability for own practice\.
+ Participates in quality improvement projects and initiatives\.
+ May act as a lead, mentor, or trainer to other staff\.
Minimum Requirements
+ Current and applicable LPN License in the state of Utah\.
+ Basic Life Support Certification \(BLS\) for healthcare providers\.
+ Approved IV Certification \(As required by facility\)\.
Physical Requirements
+ Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies\.
+ Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations\.
+ Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy\. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc\.
+ Expected to lift and utilize full range of movement to transfer patients\. Will also bend to retrieve, lift, and carry supplies and equipment\. Typically includes items of varying weights, up to and including heavy items\.
+ Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment\. This includes pushing/pulling gurneys and portable equipment, including heavy items\. Often required to navigate crowded and busy rooms \(full of equipment, power cords on the floor, etc\)\.
+ May be expected to stand in a stationary position for an extended period of time\.
